The Daily COVID-19 Health Check is no longer required by the University of Pittsburgh. This form will be removed on August 9.





As a part of the University of Pittsburgh’s ongoing efforts to reduce the spread of COVID-19, all guests and academic visitors are required to complete this daily self-attestation before reporting to (or circulating on) campus. 

The information provided will be collected, aggregated, and potentially disclosed by the University as necessary for public health and safety.

If you are experiencing any one of the following symptoms that cannot be attributed to another known health condition (e.g., seasonal allergies), stay home:
      • NEW Shortness of breath or difficulty breathing
      • Fever over 100.4°F or chills
      • Cough
      • Fatigue
      • Muscle or body aches
      • Headache
      • New loss of taste or smell
      • Sore throat
      • Congestion or runny nose
      • Nausea or vomiting
      • Diarrhea

If you have been in contact with someone known to have COVID-19 (outside of a patient care setting while using appropriate PPE), have been told by a public health official or employer that you have been exposed to COVID-19 in the past 14 days, or have anyone in your household showing COVID-19 symptoms, stay home.


If you plan to travel, please consider your risk of contracting and transmitting COVID-19. Be sure to consult the University’s guidance on travel in advance. 

If you have returned from travel, monitor yourself closely for symptoms of COVID-19. Follow the University’s quarantine requirements after travel.  
If you have responded with yes to any of the above questions you may not circulate on campus and should follow the guidance below: 
  • Stay home or return to your current residence, if you become ill while on campus. 
  • Employees must follow their departments’ procedure for calling out sick or requesting work from home. 
  • Contact your health care provider. 
  • Students should call Student Health Services at 412-383-1800. Faculty and staff should call Employee Health Services at 412-647-4949.
